The following policy applies to ChartMD Holdings, LLC's suite of web services, hereafter referred to as "ChartMD" or "Site". The phrases "we", "us", "our", "MetricsMD", "Chart Helper", and "Fast Charts" refers to ChartMD collectively. INFORMATION WE COLLECT Our Contact forms collect personal information such as names, emails, and phone numbers. We also collect IP addresses (see "USE OF IP ADDRESSES" for more information). CharMD provides a multitude of services, some of which include data entry and the importation or exportation of data to third-parties, according to the user's preference. HOW WE USE THE INFORMATION WE COLLECT We don't sell your information to anyone outside of ChartMD. We use phone numbers and email for two-factor-authentication services. Upon consent, phone numbers and email may be used with third-party SMS, email, and out-dialer systems to contact you for opt-in services, education, newsletters, reminders, billing, and notices, which you may opt-out of at any time. Some information may be shared with third parties during legal proceedings, security investigations, and billing or to government authorities when ordered to do so from a court of law. Some features allow you to voluntarily exchange data with third-parties, in which we do not have control over what the other party does with your data. In each instance, you will be informed of this when opting-in. We do not share or sell SMS opt-in or phone numbers for the purpose of SMS to third-parties. USE OF IP ADDRESSES We collect IP addresses when a user visits, logs in, posts messages, or interacts with the forms or other features of the Site.
